We love taking photos of your pet(s). We not only use these pictures to send to you, but we would love to feature your pet on our website and advertisements but we will need your permission first! Please complete the information below.
I hereby give permission to take and use photographs and videos of my pet(s) for the purpose of documenting care, services, and using on social media accounts, advertisements, and websites.
I hereby release and discharge any and all claims arising out of use of the photographs and/or videos. In the aforementioned social media accounts and website, I also give my permission to use my name and my pet’s name for the purposes of social media promotions. I also understand that no payment or compensated will be made for the photographs.
